Clinical Lab Finder is an independent website. It is not affiliated with, endorsed by or sponsored by the U.S. Centers for Medicare & Medicaid Services (CMS), the Department of Health and Human Services (HHS) or any government agency. A listing is not a recommendation.

Data sources and dates

• CMS NPPES NPI Registry (npiregistry.cms.hhs.gov), retrieved and checked 15 October 2026. Each listing shows its own verified date and a link to its source record.
• CMS Provider of Services File – Clinical Laboratories (data.cms.gov), release for 1 April 2026 (Q2 2026). CLIA details appear only where the lab's name and ZIP code match exactly.

Both are U.S. government works. CMS notes that an NPI does not show a provider is licensed or credentialed. Always check current status with the issuing authority.

Deactivated NPIs

Following CMS guidance, for a deactivated NPI we show only the NPI and its deactivation date.

Corrections policy

Source data is reported to CMS by providers. To change it permanently, update your NPPES record at nppes.cms.hhs.gov. You can also send us a correction; we aim to review it within 10 working days. We may remove a listing that is clearly not a clinical laboratory.
